What possibly could the circumstances be? Could it be a coded message?

I doubt if a coded message would draw attention to itself so luridly. So many details and no clear narrative. The volunteer status, the cleaning oif the basin, the amputation of a hand seem to be details that loom large in someone's disturbed thoughts . . .
Makes a change from those Prayers to Saint Jude.  |
